A lip flip is a small-dose botulinum toxin treatment designed to show slightly more upper lip without adding filler. It can help when the upper lip rolls inward or looks thinner during smiling. The aim is a subtle movement change, not a dramatic increase in size.
The treatment is quick and usually involves little visible downtime. However, the treated muscle also helps with speech, drinking, eating and expression, so conservative dosing matters.

How does a lip flip work?
The orbicularis oris is the circular muscle surrounding the mouth. Its fibres close and shape the lips. Small amounts of botulinum toxin placed into selected superficial fibres above the upper lip temporarily soften the inward pull.
As the muscle relaxes, the upper lip can roll outward and reveal more vermilion, the pink part of the lip. This can look fuller even though no volume has been added. A 2021 three-dimensional study reported mild enhancement, while a 2025 systematic review found broadly consistent eversion across the small available studies. Evidence remains limited compared with licensed cosmetic toxin indications.
A lip flip is an off-label use of botulinum toxin in the UK. This means a prescriber uses a licensed medicine outside its listed indications, based on clinical judgement and informed consent.
What can a lip flip achieve?
A lip flip may show slightly more upper lip, soften an inward curl and improve balance during smiling. It may also soften some dynamic vertical lines. The result should remain natural.
It cannot add true volume, define the border like filler, replace age-related tissue loss or completely correct structural asymmetry. It also cannot solve every gummy smile. Excess gum show can relate to lip movement, tooth position, gum height or jaw structure, so the cause needs assessing before treatment.

Who is a good candidate?
The treatment often suits people with an inward-turning upper lip who lose visible pink lip when smiling and want a subtle change. People whose work depends on precise lip control, including singers and wind-instrument musicians, should discuss the functional trade-offs carefully.
It may not be suitable with an infection, cold sore or inflamed skin near the injection points, allergy to the medicine, neuromuscular disorder, swallowing problem or previous unwanted weakness from toxin. Tell your prescriber about all medicines and planned dental or aesthetic procedures.
Dr Motox does not provide botulinum toxin treatment during pregnancy. If you are trying to conceive, you must be certain that you are not pregnant at the time of treatment. Breastfeeding, acute illness and significant medical conditions also require an individual discussion before booking.
When will the result appear and how long does it last?
The effect is not immediate. A change often begins within two to four days and becomes clearer over ten to fourteen days. This is why balance should not be judged in the first week. A review at around two weeks allows the movement and oral function to be assessed once the medicine has settled.
Lip movement is strong, so a lip flip can wear off sooner than forehead toxin. Fading often occurs over six to twelve weeks, although duration varies. More medicine is not automatically better because higher doses can increase functional side effects.

What are the risks and side effects?
Common short-term effects include small bumps, tenderness, redness, mild swelling, bruising or headache. Functional effects can include difficulty using a straw, whistling, pronouncing certain sounds, keeping liquid inside the mouth or forming a tight seal. The smile may look uneven if each side responds differently.
These effects generally improve as the toxin wears off, but there is no instant reversal. Rarely, toxin effects can spread beyond the intended area. Seek urgent medical advice for marked weakness, difficulty swallowing, speech changes or breathing problems. Lip flip aftercare
Keep the area clean and avoid rubbing or massaging the injection points that day. Avoid strenuous exercise, excessive heat, saunas and alcohol until the following day if advised. Delay facials or devices that press around the mouth. Apply makeup only once the injection points have closed, using clean tools.
For dryness, use a simple fragrance-free lip balm rather than an active plumping product. Hydration should continue across the rest of the routine. Will it help smoker's lines?
